Ages & Stages Questionnaire (ASQ)
The ASQ is a simple way to see how your child is growing and learning. It helps you notice what your child is doing well and spot any areas where they might need a little extra support. Since you spend the most time with your child, your answers give us the best picture of their development!
Interested in completing an ASQ?
-You complete the questionnaire at home; it only takes about 10-15 minutes.
-The ASQ asks about everyday activities like walking, talking, and playing.
-After you submit the questionnaire, our Care Coordinator team reviews your answers.
-You'll receive a personalized summary with feedback, ideas for activities to support your child, and information on next steps, if needed.
